Welcome to Bath Local Schools!  If you did not upload the required documents into your Final Forms portal, please bring these required documents to the building office:  students birth certificate or passport, social security card, vaccination records, 3 proofs of residency, and any custody paperwork or special education (IEP/504 plans/gifted identification) if applicable.  Kindergarten students will also be required to furnish a signed physical form.
