Welcome to Bath Local Schools!  Please upload the required documents into your Final Forms portal within 7 days of registering your student.  The required documents are: student birth certificate or passport, social security card, vaccination records, 3 proofs of residency, and any custody paperwork or special education (IEP/504 plans/gifted identification) if applicable.  Kindergarten students will also be required to furnish a signed physical form.
